Sometimes the biggest barrier to your health isn't anything that happens in the exam room. It's whether you have a ride to get here, food on the table, or coverage you actually understand. That's where Jonel Crispin comes in.
As MFM Health's Community Health Worker, Jonel connects patients to the everyday resources and support that keep them healthy outside our walls. Her services are free, confidential, and available to any MFM Health patient. You don't need a referral, and you don't need to have things figured out before you reach out.
Community Health Worker Services
Getting to and from your appointments
Missing an appointment because you don't have a ride shouldn't set your care back. Jonel can help you schedule transportation and walk you through any applications needed to access it, along with general help booking and keeping track of your appointments.
Food security
Jonel helps patients apply for and renew SNAP benefits, work through any issues that come up along the way, and connect with local food pantries and community meal programs near you.
Health insurance and benefits
Understanding what your health insurance actually covers, or applying for coverage in the first place, can be confusing. Jonel helps you navigate your plan and works with patients applying for Social Security benefits.
Housing and homelessness support
If you're facing eviction or need emergency shelter, Jonel can guide you through your options and connect you with the right resources quickly.
Budgeting and financial management
Jonel offers practical support with everyday budgeting and financial management, helping you build a plan that fits your circumstances.
Support at home
Jonel helps identify where extra support is needed at home and connects patients to home care, laundry, and meal prep resources, coordinating those services so you don't have to manage it alone.
Mental health resources
Jonel connects patients with behavioral or mental health resources in the community, meeting you where you are.

For older adult patients, Jonel helps identify and apply for services designed to support aging in place and independence.

Jonel's role doesn't stop at a single referral. She follows up with patients weekly, conducts outreach with patients and families, advocates on your behalf, and connects you to emergency resources whenever the need comes up.
