Schedule your appointment: Book online or call our office to set up your first visit. With 24-hour access this is often the most convenient step to getting started with us.
Fill out new patient forms: Access and complete your forms online before your visit. These forms will save you time and help us understand your health history.
Transfer your medical records: If you’re transferring from another provider, please request that your records be sent to us. Complete the request form, so we may reach out to your previous office.
Bring insurance information: Have your insurance card and photo ID ready to upload online or bring them to your visit.
Step 2: Prepare for Your Visit
Verify referral requirements: If your visit requires a referral (e.g., for specialty care), confirm with your primary care provider ahead of time.
Bring a list of everything you take regularly for your health, including over-the-counter items and daily supplements.

Arrive early: Arrive 10–15 minutes before your appointment to allow time for check-in.

Bring a payment method: We accept most major forms of payment for copays or additional services.
Step 4: Use Our Patient Tools
Sign up for our patient portal: Conveniently manage your health, access test results, and communicate with providers securely.
Link your devices: If you use health devices like a Continuous Glucose Monitor (CGM), link your data to our practice for seamless care coordination.
Utilize drive-thru services: Diagnostic testing, including COVID-19, RSV, flu, and strep tests, can be handled via our drive-thru in Middleton. Our providers will instruct you during a telehealth or in-person visit on when to use this service.
