House Calls Services on the North Shore
MFM Health’s House Calls program provides in-home medical visits for adult patients who may have difficulty traveling to the office for care. Our clinicians offer evaluation, monitoring, and coordination of ongoing health needs through scheduled in-home or telehealth appointments.
Chronic Disease Management
Our team provides evaluation and management for chronic health conditions through in-home and telehealth visits. Care focuses on monitoring, medication oversight, and coordination with your primary care team to support long-term wellness.
Annual Wellness Visits
Our in-home wellness visits include preventive screenings, health assessments, and care planning designed to support ongoing wellness and early detection of health concerns.
Urgent and Same Day Sick Visits
We provide in-home evaluation and management for acute health concerns through scheduled house calls. Our clinicians assess symptoms, recommend next steps, and coordinate follow-up care when needed.
Lab Testing
Our clinicians can collect lab samples during scheduled in-home visits, allowing patients to complete routine testing and monitoring as part of their ongoing care.
Immunizations
Our clinicians can provide vaccinations, such as influenza, pneumonia, and shingles, during scheduled in-home visits when clinically appropriate.
Wound Care
Our clinicians provide evaluation and management of wounds, including surgical incisions, pressure injuries, and diabetic foot wounds. Care may include dressing changes, documentation of healing, and recommendations to support proper wound management at home.
Diagnostics (X-Ray, Ultrasound, EKG)
Our clinicians can arrange in-home diagnostic testing, including X-rays, ultrasounds, and EKGs, when clinically appropriate. These services support timely evaluation and coordination of ongoing care.
Home Safety Evaluation
Our clinicians provide in-home safety assessments to identify potential risks and recommend modifications that support mobility, accessibility, and overall well-being.
Post-Discharge Follow-Up Visits
Our clinicians provide in-home follow-up visits after a hospital stay or skilled nursing discharge. These appointments focus on reviewing medications, monitoring recovery, and coordinating next steps in your ongoing care plan.
Skilled Nursing & Assisted Living Placement
Our team assists with coordinating transitions to skilled nursing or assisted living facilities. We collaborate with patients, families, and care teams to ensure appropriate placement and continuity of care.
Remote Patient Monitoring

Our House Calls team works collaboratively with other healthcare professionals involved in a patient’s care—such as specialists, physical therapists, and home health aides—to support communication, continuity, and coordination across all aspects of care.
