Patient Communications Program
Email and Text Messaging Terms and Conditions
Effective: August 25, 2026
At MFM Health, we strive to ensure that we provide timely and convenient communications to our patients. We have created a Patient Communication Program (Program) that allows patients to receive electronic communications via email and/or text message from us. If you agree to participate in the Program and provide your email address and/or mobile telephone number these terms and conditions explain how MFM Health may communicate with you.

Consent to receive email or text messages is not a condition of receiving healthcare services from MFM Health and you may opt out of the Program at any time.

Communications from MFM Health
Through the Program, communications you receive may vary based on your appointments, care, account activity, preferences, and interactions with MFM Health as follows:

Appointment confirmations, reminders, cancellations, and scheduling information

Registration, check-in, forms, and administrative instructions

Preventive care and routine follow-up reminders

Notifications that it may be time to schedule a visit, screening, test, or other service

Patient portal and account notifications

Practice hours, closures, locations, services, and operational updates

Health education, newsletters, events, and other informational communications

Privacy and Security
If you participate in the Program, you should know that email and text messaging may not be fully secure. Messages may be accessed by anyone who has access to your email account, telephone, mobile device, wireless plan, or message notifications.

MFM Health takes reasonable steps to limit the sensitive information included in standard email and text messages. However, the content of a message, the identity of the sender, or the fact that you receive messages from MFM Health could reveal information about your relationship with the practice.

You are responsible for considering whether email and text messaging are appropriate for your circumstances and for protecting access to your accounts and devices.

Please do not send detailed medical information, photographs, financial information, Social Security numbers, passwords, or other highly sensitive information by standard email or text message.

When secure communication is appropriate, MFM Health will direct you to use the patient portal or another approved communication method, including the patient portal.

MFM Health does not share mobile telephone numbers or text messaging consent information with third parties or affiliates for their marketing or promotional purposes and does not sell mobile telephone numbers, email addresses, text messaging consent information, or opt-in data.

MFM Health may share information with vendors and service providers that help us deliver communications and operate our services. These organizations may use the information only to provide services to MFM Health or as otherwise permitted or required by law.
Medical Emergencies and Urgent Concerns
Email and text messaging should not be used for medical emergencies or urgent medical concerns. Messages may not be reviewed immediately, and delivery or response is not guaranteed. Call 911 or go to the nearest emergency department if you are experiencing a medical emergency. For urgent medical concerns that are not emergencies, call MFM Health at 978-774-2555 and follow the available instructions.

Do not rely on an email, text message, automated response, or delivery confirmation as evidence that MFM Health has received or reviewed an urgent request.
No Medical Advice Through General Messages
General email and text communications from MFM Health are provided for informational, scheduling, administrative, or care-coordination purposes. A general message does not establish a diagnosis, replace an examination, or constitute individualized medical advice unless the communication is specifically provided by an authorized member of your care team in an appropriate clinical context. Contact your healthcare provider through the patient portal with questions about your individual care.
Text Messaging
a. Message frequency
Message frequency varies based on your appointments, care needs, account activity, communication preferences, and interactions with MFM Health. MFM Health does not guarantee a specific number or frequency of messages. Many messages will be automated.
b. Message and data rates
Message and data rates may apply based on your mobile telephone plan. MFM Health does not charge a separate fee for receiving text messages, but your wireless carrier may apply standard messaging, data, or other charges.
c. How to opt out of text messages
You may opt out of text messages by replying STOP to an MFM Health text message.

MFM Health may send one final text message confirming that your opt-out request has been processed. After that confirmation, you will no longer receive text messages from that messaging program unless you provide consent again.

MFM Health will also honor other reasonable requests to stop text messages, as required by applicable law. You may contact MFM Health at 978-774-2555 for assistance with your communication preferences.

Opting out of text messages does not prevent MFM Health from contacting you through other permitted methods, including telephone calls, email, the patient portal, or mail.

Opting out of one type of communication may affect other text messages sent by MFM Health. Please contact us if you need help managing specific communication preferences.
d. How to get help
Reply HELP to an MFM Health text message for assistance, or call MFM Health at 978-774-2555.
e. Changing your mobile number
You are responsible for providing MFM Health with an accurate and current mobile number.
Please notify MFM Health if:

Your mobile number changes

Your mobile service is disconnected

You transfer your number to another person
-
You are no longer the authorized user of the number
You agree that MFM Health may continue sending messages to the number provided until we receive and process notice that the number has changed or is no longer associated with you.
f. Mobile carriers and delivery
Wireless carriers are not liable for delayed or undelivered messages. Message delivery is subject to effective transmission by your wireless carrier and is outside MFM Health's control. Messages may be delayed, blocked, filtered, misdirected, or not delivered because of carrier limitations, device settings, network availability, technical issues, or other circumstances. MFM Health does not guarantee that every text message will be received.
Email Communications - Opting Out / Unsubscribing
Promotional and informational emails will include an unsubscribe link when required.
You may use the unsubscribe link in an email to stop receiving the applicable category of promotional or informational email messages.
Unsubscribing from promotional or newsletter emails may not stop communications that are primarily related to:

Existing appointments

Care coordination
-
Provider or location transitions

Patient safety
-
Forms or registration
-
Account security
-
Billing or insurance
-
Medical records
-
Services you requested

Other non-promotional aspects of your relationship with MFM Health
MFM Health may continue sending non-promotional communications when permitted by law and appropriate for your care or relationship with the practice.
You may also contact MFM Health at 978-774-2555 for assistance with your email preferences.
Permission to Use Your Contact Information
You represent that you are the subscriber, owner, or authorized user of mobile number and/or email address you provide to MFM Health. If you provide contact information for another individual, including a family member, dependent, guarantor, or caregiver, you represent that you are authorized to provide that information and to receive communications as permitted by applicable law. Providing shared or family contact information may allow other people with access to that email account or mobile device to see communications intended for you.
Third-Party Service Providers
MFM Health may use third-party vendors, telecommunications providers, email platforms, messaging platforms, and other service providers to deliver and manage communications. Your wireless carrier, email provider, device manufacturer, internet provider, or other third party may apply its own terms, privacy practices, restrictions, or charges. MFM Health does not control and is not responsible for those third-party practices.
Links in Messages
Emails and text messages may contain links to MFM Health websites, scheduling tools, forms, the patient portal, payment services, educational resources, or other websites. Use caution before selecting a link in any message.
MFM Health will not ask you by text message or unsecured email to provide your password, full Social Security number, or complete payment card information. Contact MFM Health directly at 978-774-2555 if you are unsure whether a message is legitimate.
Changes to These Terms
MFM Health may add, change, suspend, or discontinue the Program at any time. MFM Health may also change the types of communications offered through the Program or require you to provide updated consent or contact information. Updated terms and conditions will be posted on this page with a revised effective date. Your continued participation in the Program after updated terms take effect constitutes acceptance of those terms to the extent permitted by law.
